Palliative Care

Physicians usually recommend palliative care at the diagnosis of a serious, chronic or life-threatening illness. It is designed to improve the quality of life of patients and help them manage their disease or illness. Our person-centered approach can help those living with congestive heart failure, cancer, dementia, as well as many other diseases and chronic conditions.

Personalized Care Plans

Palliative care is about supporting the unique needs and goals of individuals and their families. At Right at Home, we develop personalized care plans—these plans are developed in accordance with guidelines and recommendations offered by your palliative care coordinator.

To access our program, you should begin by becoming a part of the Winnipeg Regional Health Authority (WRHA) Palliative Care Program. Our team can answer any questions you might have about the WRHA Palliative Care Program.

Our personalized care programs can include:

  • Transportation to and from appointments
  • Coordinating medical care
  • Meal preparation
  • Running errands
  • Bathing and showering assistance
  • Bed baths
  • Grooming
  • Oral hygiene

And more

Medical Support And Symptom Management

Medical support and symptom management are largely managed by nurses, doctors, and other medical staff. Our team can help coordinate medical support and care with health care professionals from the WRHA and its specialized palliative care units.
